Ontario’s Statutory Accident Benefits Schedule runs on standard forms. Which ones a file needs depends on facts about the accident and the client — facts that are all collectable in the first phone call, if the right questions get asked.

The forms, briefly

  • OCF-1 — Application for Accident Benefits. Every file starts here.
  • OCF-2 — Employer’s Confirmation. Needed when income replacement is on the table.
  • OCF-3 — Disability Certificate. Required when injuries affect the ability to work or live normally.
  • OCF-5 — Permission to Disclose Health Information.
  • OCF-6 — Expenses Claim, for out-of-pocket costs.
  • OCF-10 — Election of benefits where more than one applies.

Rules, not guesswork

Whether a file needs an OCF-2 or an OCF-3 is not a judgment call — it follows deterministically from facts like “did the client miss work?” and “was a disability reported?”.
